one hand stabilizes the elbow while the other hand squeezes across the distal biceps muscle belly. a positive test is failure to observe supination of the patient’s forearm or wrist. The squeeze test was developed by Ruland et al13 and is per-formed with the patient seated and the forearm in 60e80 of flexion as it is placed in his lap. The forearm is slightly pronated. The investigator stands on the lateral (affected) side of the patient and grabs the biceps with both hands, with the thumbs at the The squeeze test has been found to have moderate reliability (kappa = 0.50) according to a study performed by Alonso et al. and according to the study done by de César PC et al, the test has a sensitivity of 30% and a specificity of 93.5% for a high ankle sprain. Posterior Drawer Test Elbow is used for Lateral Ulnar Collateral Ligament Injury (Posterolateral Rotatory Instability) 5.

O’Driscoll SW The examiner hooks her or his index finger laterally around the distal biceps tendon to demonstrate its presence. The biceps squeeze test is similar to the Thompson test for Achilles tendon rupture. With external compression placed on the biceps muscle, the forearm is supinated if the biceps tendon is intact. The hook test was performed by a single surgeon in a cohort of 45 patients undergoing surgical exploration of the distal biceps tendon. While the patient actively supinates with the elbow flexed 90°, an intact hook test permits the examiner to hook his or her index finger under the intact biceps tendon from the lateral side. The hook test was performed by a single surgeon in a cohort of 45 patients undergoing surgical exploration of the distal biceps tendon.

If the distal biceps tendon is intact, there is substantial change in the shape of the biceps as the arm is supinated (the biceps moves proximally), then pronated (the

elbow held in 60-80° of flexion with the forearm slightly pronated.

Another test to diagnose the distal biceps tendon rupture is the Squeeze Test (Figure 4), which is similar to the Thompson’s Test—a method used to diagnose an Achilles tendon rupture. For the Squeeze test, you will ask the patient to flex their elbow to 80 degrees and keep the forearm in some pronation, then you will squeeze the biceps with one hand or with two hands. Having a systematic and structured approach to the shoulder history and examination ensures that key aspects of the condition are elicited and important conditions are not missed. Information gathered in this process can help guide decisions about the need for special tests or investigations and ongoing management.

The biceps squeeze test is simple, reliable, cost-effective, and aids in the diagnosis of distal biceps tendon ruptures. Level of evidence: Diagnostic study, Level II-1 (development of diagnostic

Twenty-two of the 24 patients with a positive biceps squeeze test had surgical repair of their distal biceps tendon rupture. In 21 of these 22 patients (95%), surgical exploration confirmed complete distal biceps tendon rupture. One patient had a 50% disruption of the distal biceps tendon that was not reconstructed.
